Every customer is different, so the support and help that you provide will depend on your customer’s needs.
The successful applicant’s duties although the list is not exhaustive will include the following:-
To carry out initial assessment with an individual of their health, social care, housing and community needs.
To build up good networks and working relationships with a range of services and professionals
To investigate potential resources in the community and maintain good knowledge of local and City-Wide
To agree, review and monitor individual programmes with professional services using agreed processes
To communicate effectively with users, carers and staff and other professionals using agreed systems of communication
To record user information, maintaining and updating files and electronic systems and preparing and producing reports
To participate in relevant training courses
To work as part of a team and contribute to team meetings
General duties involve providing team members with high quality support to enable effective service delivery, and to undertake any other duties in agreement with the post holder and manager.
Successful candidates will be required to complete an enhanced Disclosure and Barred Service check
